Registration Station
Registration is closing for the 2009-10 BAHA Season. (Updated 12-11-09PM) Registration has been open since mid-September. It will be closing on December, 15th, 2009. Registration information, needed to complete an existing registration, will still be accepted. It can be mailed, or dropped off. The address will be supplied, after you send an e-mail to berrienareahockey@gmail.com . Be sure to review the Required Registration Information list below, prior to attending. Players will not be registered, without all of the necessary documentation.
Required Registration Information (Updated 10-11-09PM) STEP #1 - USA Hockey Registration - All players need to register directly with USA Hockey, via their website. Review "Quick Tips for USA Hockey Website", then use the link below. This will take you directly to their website. This must be done before coming to the BAHA registration dates.
Quick Tips for USA Hockey website - This registration is a $35 non-refundable fee payable with Credit Card. - Select “Ice Players & Coaches” - Users must be 18 to register on-line. When prompted, check the box and continue… - You will be registering for the 2009-10 season. - When prompted, select “Myself, Family Member Under 18, or Non-Family Member Under 18” and continue….
- YOU MUST USE THE LEGAL NAME OF THE PERSON YOU ARE REGISTERING – It must match what is on the skater or coach’s birth certificate. If the person being registered is a Jr. or a III, then that must be entered, also. It would be entered in the last name field after the surname. Do not use nicknames. If your surname has an apostrophe in it, (such as O’Toole) you cannot use that in this registration program. Use a space in place of the apostrophe.
- The birth date MUST match the birth certificate. Double-check this entry.
- When you are finished completing all of the information, read the waiver of liability portion of the registration, accept the waiver, and enter YOUR initials where noted.
- You must print the confirmation of your registration and bring it to the BAHA Registration Dates.
STEP #2 - Download, Print, & Complete Forms. The following links will access the 3 required forms. Some documents require player and parent signatures. All paperwork must be complete. Registration Forms: (click on Form name)

* Participant Consent to Treat Form * Parent / Guardian Code of Conduct Form * Southwest Michigan Freeze Registration Form (Not for BAHA House /Rec League) STEP #3 - All new and returning players need to bring a copy of the players Birth Certificate. (Hospital Certificates are not acceptable) BAHA will have to keep a copy of the Birth Certificate. STEP #4 - Look for each players birth year, in the BAHA Fee Schedule. Payment can be made by check, or cash. All documents and payment should be brought to the registration dates. Information can also be sent to the above address. Please only mail checks. Do not mail cash.
